Welcome to DM² New Brunswick
The goal of DM2 NB is to improve the health of our New Brunswick patient participants with type 2 diabetes by slowing or preventing the progression of the disease and its related complications such as kidney disease, heart conditions, dialysis, amputations, increased medication costs, reduced quality of life.
The multi-system disease management approach, if proven effective, could provide a model for future disease management strategies in New Brunswick and across North America.
Hypothesis: Implementation of the described a multi-system disease management two-fold model will improve blood pressure, slow or halt the progression of kidney disease and improve the management of blood sugar in patients with diabetes.
Primary Outcomes: Reduction in blood pressure, reduction of GFR decline (ml/min/yr) and reduction in AIC
Secondary Outcomes: We will also evaluate whether or not this model can also improve cholesterol, prevent worsening of kidney damage and the need for dialysis, reduce the incidence of overall death, reduce cost of care, and improve satisfaction with care by patient and physician.
